What is an Advance Directive

Advance Health Care Directive, is a legal document, where adults over the age of 18, like you, designate another party to act on your behalf during a medical situations that limit or prevent you from communicating. It gives hospitals, physicians and others a go to authority to quickly and efficiently make decisions that can save your life.

I'm Young; isn't an Excuse

In the United States, the most notable court cases actually involved young adults. Remember Terri Schiavo? She was just 26 years old when decisions had to be assumed regarding her health care rights and ultimately her life. Stating your health care preferences, signing an advance directive, asking your physician for orders regarding life support and registering these documents may feel permanent. It's not. And your decisions can and likely will change throughout life. Unfortunately, according to British Medical Journal, avoiding this "adult" task results in higher levels anxiety, post-traumatic stress and depression on your family and friends.

Helping Individuals and Families

Our mission is to empower others through a balanced use of innovation, empathy and resources. Since COVID, individuals have sought companies willing to meet the needs of remote access. We focused on that goal in developing ways to serve our beloved neighbors remotely and privately. Today, our clients can complete their documents from the security and comfort of their homes, without the pressure of others, while having the option to integrate with their normal health care providers, if needed.

Case Studies

Barriers: Lack of Comfort

Individuals are often uncomfortable with Advance Care Planning. Personal experience with death can encourage patients to consider the circumstances around their own end-of-life care, but a poor understanding of disease trajectory and discomfort with discussing death could prevent people from having Advance Care Planning. Even when individuals and their families are interested in Advance Care Planning, they sometimes feel uncomfortable initiating the conversation themselves, especially with practitioners.

Impact of Advance Care Planning

A coordinated, systematic model of patient centered advance care planning using non-medical advance care planning facilitators assists in identifying and respecting patient's wishes about end of life care, improves such care from the perspective of the patient and the family, and diminishes the likelihood of stress, anxiety, and depression in surviving relatives.

Partnerships

Working Together

Through our mission of empowering our Community we work closely with providers, hospitals, assisted-living facilities and others within our health care sector. Client documents can be accessed 24/7 by our registered and verified partners in an emergency. Our clients can also provide others with their document identification number and pin allowing direct access by their family, friends and other health care partners.

Compliance

Our system uses industry standards for both encryption and development. To help our partners with their own internal and regulatory compliance, we also provide utilization reporting. This limited reporting is related solely to amount of time our application is used but will help providers comply with Medicare, Medicaid and other private insurance companies, policies, and regulations.
